In a nutshell, the Federated Model with RBAC works like this: when you build a Health Information Exchange (locally, statewide, nationally) you need one security model with credentialed users. These users have different roles, and based upon their roles in the HIE (or organization), the users will have different access controls (ie, a physician sees different data than a billing clerk, etc.).
